Yes these are common failures for the Sony monitors. 

Yes you have to get the adapter cable. 

On my rig there is an access panel on the dash in front of the steering wheel that is removable, then you can remove the nut on the left side, you will have to trim the panel just a bit to install the née monitor RV-Camer’s will request the you send the a photo to make sure their unit will fit they are very helpful and I recommend them highly!!!

It sounds like the monitor video screen has gone out, I have a 2008 Dynasty change mine out about 5 months ago  contact RV Cameras in Baton Rouge, La they can help you they have as close to and extant replacement at you can get as the don’t make the Sony any longer.  The unit is a little over $400.00. 

Bob good morning, I just recently change mine out on my 2008 Monaco Dynasty Squire, after trying to find the controller switches to no avail. I ordered a 40450 Southwire Transfer Switch through Boat & RV Accessories they had the best price $ 671.00 free shipping it was easy to install and it was almost the identical unit with the exception of a few power cord connections. I receive in about a week, I just ordered the remote plug in unit that lets you know is the voltage is high/low or the polarity is incorrect from shore power. Roger, my name is Jim Clendenon I’m the one that purchased all of these items, my forward A/C is out I’m hopping the the circuit board corrects the problem, if so I may not need the control box I’ll know in a few days if so would you still be interested in same???

I second RV Cams, I purchased replacement cam for the Sony dash monitor for my 2008 Monaco very helpful they know their stuff.
